The mafia do not submit a kill of a role they submit an action to kill a player. The only role that passively protects anything is the TE who bodyguards the DST and dies instead of the DST. And this creates a scenario where the doctor cant self target themself to survive either. There is no benefit to electing someone to be both doc and TE.

If doc is targeted TE dies thus if TE is town maf get in a double kill. This means QB is kind of obliged to watch doc to prevent this. And doc probably saves QB or Flex (provided not in both roles).

So, as I said ages ago the NK will almost certainly be from the bench or positions 2 or 3.

Roster management should take this into account.

If the doc can prevent the TE from dying in their stead, then we have an infinite protection glitch in addition to the superpower.

The TE will die in this example unless the FLEX item was the vest.
The rationale is because the player was targeted who happens to be these various roles; since the DST cannot die and the TE dies in their place; the doc would survive and TE would perish.’
Mafia are targeting a player not a role; thus the only role here that triggers any role based response to dying comes into effect - the Bodyguards TE saving of the DST.

The bodyguard can be docsaved.
The Doctor and Bodyguard can function in the way youve described.

I do not believe this should be classified as a glitch; but if you feel that combination is game breaking I will allow you to have that opinion.
