They would appear town at for first, but it would let the doc and the saved person know they were mafia, so I'd imagine it's a bad scum play

I don't see any reason for Brain to fake this as the Doc can say he is lying. So Brain is a clear for all practical reasons.

The Doc can save themselves. But the Doc also can not save the same person twice in the row.

Still Suspicious – I mean, I know you said you are skimming, but you are asking a lot of questions that have already been asked and answered. At the very least that makes BK3K a weaker link Townie whom I presume Mafias would be all to happy to have around longer.
