Is one spelling an English spelling and an American spelling? Or are these two spellings used interchangeably?

No, it is not a difference of American and English spellings. The shorter version of the first word is less formal than the longer version. I will note that the longer version of the entire phrase is approximately 34.5x more popular than the shorter version according to Google Trends.

I'm back to Doughnut (or Donut) holes. It fits EVERY clue. Even the 'not food.' Because a hole by definition has NOTHING in it.

I would again like to point out that Doughnut holes is the closest guess we've had so far in terms of substance.
Yes, in the short version of the first word, the second word is 2 letters longer than the first word.
