I don't think that the list covers all possibilities. Self reporting tends to always have a degree of subjectivity and is rarely 100% objective. I would describe my behaviour with the Black Art of stabbing in Diplomacy as:
I try to avoid the necessity of a stab by using Diplomacy or settling for a Draw. There have to be sound reasons for me to stab. I might give a "veiled warning", but generally will.never give an explicit warning. A more interesting part of this discussion might be on how we behave when we are the victims of a stab. I'm usually extremely emotional, will usually be hostile and abusive to the player who stabbed me and will also usually refuse to fight back, I will disband a retreating unit rather than retreat and often leave the game (hence my wonderful reliability rating)

No warning in late game stabs but I declare war in first year generally so people see how honourable I am and they more likely accept me as an ally when I need this. That's something great to see their reaction to your war declaration. I don't know I'm not a good player and still trying new styles but this is my current way.
